Executive Health Check UK 2026: What's Included, Cost & Is It Worth It?

Executive health checks are the most-Googled preventative-health product in the UK, and one of the least well-designed. The £2,500 package with the impressive PDF often misses the two tests that would have genuinely changed your risk profile. This guide covers what actually earns its place in a modern preventative panel.

What a well-designed check should include

    Cardiovascular (the single highest-yield area)

    • Full lipid panel including ApoB
    • Lp(a), one-off test, genetically determined
    • hs-CRP
    • Resting ECG and blood pressure
    • Coronary artery calcium (CAC) score for men over 40 / women over 50, or family history, the best single scan for asymptomatic cardiovascular risk

    Metabolic

    • HbA1c, fasting insulin, fasting glucose
    • Full liver panel including GGT
    • Waist-to-height ratio (better than BMI alone)

    Cancer screening (age-appropriate only)

    • FIT test annually from 45
    • PSA + digital exam (men from 50, or 45 with family history)
    • Cervical screening + HPV (women per NHS schedule)
    • Low-dose lung CT (heavy-smoker history)
    • Skin check by a dermatologist if mole history

    Nutritional & hormonal

    • Ferritin, B12, folate, vitamin D
    • Thyroid (TSH, free T4)
    • Testosterone (men 40+), oestradiol/FSH (women 40 to 55)

    Lifestyle & functional

    • VO2 max estimate (CPET or sub-max test)
    • Grip strength
    • DEXA for body composition + bone density (women 45+, men 55+)

    What's usually oversold

    • Whole-body MRI for asymptomatic adults, the British Society of Radiologists specifically warns against routine use; incidental findings drive downstream anxiety and unnecessary biopsies
    • Genetic wellness panels without clinical genetics interpretation
    • CT colonography as a first-line. FIT is better and cheaper
    • Full-body PET, no place outside oncology
    • Extensive tumour-marker panels (CA-125, CA 19-9 etc), high false-positive rates in healthy adults

    Fair 2026 pricing

    TierFair rangeWhat you should get
Foundation£250, £450Comprehensive bloods, ECG, clinician review
Standard executive£600, £1,200Adds CAC score, DEXA, CPET, hormone panel
Consultant-led premium£1,400, £2,400Adds specialist reviews (cardiology, endocrinology), tailored follow-up plan
    Whole-body MRI-led£2,500, £4,500Rarely justified for asymptomatic adults

Companies including Bupa, Nuffield, HCA and Randox all sit in the standard-to-premium bands. Independent consultant-led clinics often match the premium content at 60 to 70% of the price because you're not paying for a branded scanning suite.

    Frequency

    • 30s (healthy, no family history): every 3 to 5 years
    • 40s: every 2 to 3 years
    • 50+: every 1 to 2 years, tuned to previous findings
    • Any positive family history: bring the schedule forward by a decade

    Getting your money's worth

    1. Ask upfront: "Which of these tests would you skip if I told you my budget was £500?", a good clinic answers honestly.
    2. Insist on a written follow-up plan, not just a PDF of numbers.
    3. Ensure the report goes to your NHS GP for the medical record.
    4. Repeat testing on a schedule matters more than a single premium check.

    FAQs

Does insurance cover this? Most PMI plans exclude routine screening. Vitality and some Bupa corporate schemes include a subsidised annual check.

Is CAC scoring safe? Radiation dose is roughly the equivalent of 6 months of background radiation, safe as a one-off in adults over 40.

Can I DIY this cheaper? Largely yes, if you have a clinician to interpret. Our private blood test guide covers the DIY route.
