Weight Loss Injections UK 2026: Mounjaro, Wegovy, Cost & Private Access
GLP-1 receptor agonists, semaglutide (Wegovy, Ozempic) and tirzepatide (Mounjaro), are the most effective non-surgical weight-loss treatments ever licensed. But the UK regulatory landscape shifted sharply in 2024 and again in early 2026. This guide covers what's legal, what's safe, and what fair pricing looks like this year.
What's available and what it does
| Drug | Trade name | Average weight loss (68 weeks) | Route |
|---|---|---|---|
| Semaglutide 2.4mg | Wegovy | 14.9% | Weekly injection |
| Tirzepatide 5 to 15mg | Mounjaro | 20.9% | Weekly injection |
| Liraglutide 3mg | Saxenda | 6 to 8% | Daily injection |
Mounjaro is now the default first-line for most private patients on cost-effectiveness and tolerability.
Who's eligible
NHS (via Tier 3/4 weight management services): BMI ≥35 with a comorbidity, or BMI ≥40. Wait times are 12 to 18 months in most ICBs.
Private (per MHRA and GPhC 2026 guidance):
- BMI ≥30, or
- BMI ≥27 with at least one weight-related comorbidity (type 2 diabetes, hypertension, sleep apnoea, dyslipidaemia, NAFLD)
- No history of MTC, MEN2 or pancreatitis
- Not pregnant, breastfeeding, or planning pregnancy within 2 months
Since April 2026, the GPhC requires photo ID verification and a documented video consultation before any first prescription. Text-message-only "consultations" are no longer lawful.
Fair 2026 pricing
| Drug | Monthly starter dose | Monthly maintenance | Legitimate private range |
|---|---|---|---|
| Mounjaro | £140, £180 (2.5mg) | £190, £260 (10mg) | Includes clinician review |
| Wegovy | £150, £195 (0.25mg) | £230, £320 (2.4mg) | Includes clinician review |
Add £40, £120 for the initial consultation. Prices under £120/month for full-dose Mounjaro almost always exclude clinical monitoring, a false economy.
What safe monitoring looks like
- Baseline: HbA1c, thyroid, lipids, LFTs, U&Es, pregnancy test where relevant
- Every 3 months: weight, side-effects review, dose titration
- Every 6 months: repeat bloods, muscle-mass check (DEXA or bioimpedance)
- Structured nutrition and resistance-training plan, muscle loss is the biggest under-discussed side effect
If your provider isn't checking bloods and doesn't ask about strength training, they're not delivering safe care.
Red flags
- No named prescriber (GPhC registration should be listed)
- Willingness to prescribe below BMI 27
- No baseline bloods before first injection
- Selling "compounded semaglutide", illegal in the UK
- Auto-shipping without review after 3 months
Coming off the drug
Roughly two-thirds of weight lost is regained within a year of stopping GLP-1s without a structured plan. In 2026 most reputable clinics build in a 6 to 12 month taper and maintenance protocol with quarterly reviews.
FAQs
Will my GP prescribe it? Only via NHS Tier 3/4 services. GPs cannot issue Wegovy or Mounjaro directly.
Can I get it on private insurance? Bupa and AXA now cover Wegovy for BMI ≥35 with a specialist referral; Vitality does not.
Is it safe long-term? Trial data extends to 3 to 4 years with no new safety signals. Real-world data is now well over 8 years for semaglutide.
