More than 172,000 adults are on an NHS autism assessment waiting list in England, and the average wait has climbed past two years, with several trusts quoting four. Going private is now the default route for most working-age adults. This guide explains what a legitimate adult autism assessment looks like in 2026, what it should cost, and how to make sure the report is accepted by your GP, your employer and the DWP.

What counts as a valid autism assessment

NICE guideline CG142 and the National Autism Strategy set the standard. A defensible adult diagnosis in the UK should include:

• A developmental history covering childhood (ideally with an informant such as a parent or sibling)
• A structured ADOS-2 (Module 4) observation, or ADI-R interview, delivered by a trained clinician

• The developmental evidence and who provided it
• What was ruled out, not only what was found
• Functional impact in plain terms: work, study, sensory environment, communication
• Recommendations you can hand to an employer, a university or a GP

A report that omits the tool used or the differential screen is the one most likely to be questioned later.

Autism and ADHD together

Because 30 to 50% of autistic adults also meet ADHD criteria, a combined assessment usually costs less than two separate ones and avoids a second developmental history.
