    Who we're looking for

    • Health trainers or health improvement practitioners
    • Strong local knowledge and signposting instincts
    • Experience working with people facing health inequalities
    • Patient, non judgemental, and comfortable with slow progress

    Qualifications and registration

    • Health trainer or health improvement qualification (Level 3 or above)
    • Brief intervention and motivational interviewing training
    • Safeguarding training appropriate to the client group
    • Enhanced DBS where the role requires it

    Responsibilities

    • Agree a personal health plan with one primary behaviour at a time
    • Deliver regular one to one sessions and follow up between them
    • Signpost to smoking cessation, activity and community services
    • Record progress against agreed, measurable goals
